Routine Health Care or Sick Pet Appointments:

New clients should arrive 15 minutes prior to their appointment time to complete new client/patient information forms. They should bring copies of their pet’s previous health vaccination records and referral letters. New client and patient registration forms are available on the home page.

Returning clients do not require new client material for existing pets, however, upon their arrival they should update their phone and address information. Returning clients with new pets will need to fill out a new patient information form.

Surgical and Hospitalization Procedures:

For the health and safety of all animals admitted to our hospital, we recommend that all pets have the following immunizations current:

 Dogs: Rabies vaccination and Canine Distemper/Parvovirus/Parainfluenza (DHPP)

 Cats: Rabies vaccination and Feline Distemper/Respiratory Viruses (FVRCP)
